AccentCare's Care
Management Plus program provides professional guidance, consultation,
planning, oversight and service coordination to assist seniors in
maintaining their independence at home and allowing family to be family.
Beginning with a comprehensive assessment, AccentCare's
Geriatric Specialists bring together the senior's needs, interests,
and circumstances to provide a highly personalized plan of care. Ongoing
reassessment and oversight offers families the peace of mind that their
loved one is maintaining a safer and fuller life while being independent at home.
Care Management Services include but are not limited to:
- Care management assessment, family consultation, and written plan of
care
- On Call Professionals available 24 hours a day, 7 days a week
- Home safety assessment and monitoring
- Arranging for and monitoring non-medical home health care, home health
nursing, durable medical equipment, and home modifications
- Medication review and reminders
- On-site monitoring on a daily, weekly, or as needed basis in person or by phone
- Advocacy services by Geriatric Care Manager
- Coordination and communication with physician
- Social activity management
- Developing plan to increase senior's independence and involvement in
community activities
- Nutritional assessment, counseling, meal planning in conjunction with
attendant & personal caregiver and physician recommendations
- Monitoring and recording vital signs
- Crisis intervention and management of complex family situations
- Recommending and coordinate with financial services providers
- Recommending and monitor contractors to manage home and garden
maintenance and repair
- Assess and present options for alternative living placement
- Discharge and transfer planning
- Disaster and contingency planning
- Crisis management, prevention and intervention
- Coordination of transportation needs
- Coordination with Long Term Care Insurance providers
- Travel planning and coordination
