The members of our Home Health care team ensure a smooth and safe transition home from a hospital stay, and provide professional nursing and rehabilitative therapies through evidence-based protocols to help avoid emergency room visits and unplanned re-hospitalization.

What to Expect

In-Home Assessment

First, a clinician will complete a comprehensive, in-home assessment to fully understand your specific needs, physician’s orders and established plan of care.

Your Care Team

Together, you will review the AccentCare Alert Your Nurse® program to  ensure you and/or a family caregiver understand how to monitor your specific condition(s) and how to reach your nurse if needed.

Peace of Mind 

Our highly-skilled, licensed clinicians participate in over 200 mandatory learning modules and continued education to qualify to deliver care, and are background-checked, worker’s compensation-insured and bonded, to not only ensure your security, but to deliver the high-quality care you deserve.

Home Health services include in-home nursing and therapy services for those recovering from illness or surgery, or managing a chronic condition or pain – at home.

“Home” means where you live. This could be a personal residence, assisted or independent living community, nursing home or wherever you call home.

We work with you and your physician to create a care plan to help you optimize your health and independence wherever you call home. With the additional goal to reduce unplanned emergency room visits and re-hospitalization, a plan of care is developed and tailored to meet you your individual needs.

Home Health services are for those recovering from illness or surgery, or need assistance managing a chronic condition or pain from home.

To be eligible for home health care, a physician must certify that an individual is in need and is homebound.

“Homebound” does not mean bed-ridden, it means that a patient can not currently leave the home due to illness or injury without “considerable and taxing effort”.

AccentCare Home Health Care services are provided by licensed professionals who participate in over 200 mandatory learning modules and continued education courses in order to deliver care. All of whom are trained and background-checked for your security and peace of mind.

Payment options may include private pay, private insurance, workers’ compensation, school districts, federal insurers, long-term care policies, trust funds or Medicaid (known as “Medi-Cal” in California).
