Behavioral Health

For patients who are appropriate for cancer care specialty services, the clinical team works closely with a patient’s oncologists and all other physicians to develop care plans, coordinate care, and avoid duplication of service.
Cardiac Care
For patients with congestive heart failure (CHF) often suffer from symptoms that interfere with daily life, such as shortness of breath, fatigue, and weakness. plans.

For patients living with COPD (Chronic Obstructive Pulmonary Disease) can require daily management. Our RightPath® COPD program can help manage symptoms and improve ambulation.
Dementia Care
AccentCare can help address dementia behaviors, while helping to ensure a safe environment to help you or your loved one remain in the comfort of home for as long as possible.
Diabetes Care
We understand that diabetes requires constant monitoring and management. Diabetes can often lead to other medical conditions such as hypertension and COPD, which are leading causes for re-hospitalization.
Infusion Therapy
AccentCare provides in-home care that is more individualized and cost-effective than an in-patient hospital stay. The clinical team works directly with physicians and infusion pharmacists to administer the prescribed therapy while monitoring effectiveness.
Late Life Depression
Our RightPath® Late Life Depression program has helped to reduce symptoms of depression and anxiety for more than 84% of our patients.
Maternal Child Care

Supports preventive health, post-natal care, including breastfeeding and child development education.
Pain Management | Palliative Care
When a patient has an advanced or chronic illness they may have symptoms or pain that their current treatment is not addressing. The focus of palliative care is to complement the patient’s current treatment in order to alleviate these symptoms and pain to enhance their quality of life.
Respite Care
Respite care helps family caregivers to get much-needed rest or time to travel for important events. Patients may receive up to five consecutive days of inpatient care at a Medicare-approved nursing facility or hospital. Respite care is meant for occasional use if part of the plan of care.
Stroke Care
AccentCare can help create customized care plans to help patients recover from strokes with in-home care. We provide a variety of therapy and nursing services to address the unique and complex needs of each patient.
Wound Care
AccentCare focuses on the treatment of wounds and improving outcomes, as well as the patient’s overall health to achieve optimal healing at home by following the evidence-based treatment protocols based on standards of care.

Rehabilitative Therapy | Orthopedic
For patients who have recently undergone joint replacement, AccentCare has developed the RightPath® Program for Joint Rehabilitation to help address their specific postoperative needs. This program has reported improved outcomes for a higher percentage of patients than the national average.
