What to expect
A licensed clinician will visit the patient at home to complete a thorough assessment based on the physician’s orders and care plan. The clinician will review the AccentCare Alert Your Nurse® program with the patient and caregiver to ensure they understand how to monitor the patient’s condition and contact the nurse when needed. All clinicians are highly trained through extensive education, background-checked, insured and bonded — providing the security and quality care patients deserve.

Who is eligible for home health care?
To receive home health care, a physician must certify the patient needs the care and can’t currently leave home without “considerable and taxing effort.”
What qualifies as “home”?
“Home” is simply where you live or where you are staying — a personal residence, senior living community or wherever you call home.
Can I ask for AccentCare?
Yes. The right to choose your health care provider is protected by law. Just ask your doctor or discharge planner about home health care services, and let them know you’d like to use our services. We’d be honored to care for you.
