Texas Home Health and Sound Telemedicine Join Partnership

AUSTIN, TX — Texas Home Health, a leading post-acute care company, and Sound Telemedicine, a division of Sound Physicians, a well-known provider of physician services, have partnered to provide enhanced access to care for patients transitioning from hospital to home health care without a known primary care physician (PCP).

When Texas Home Health receives referrals from hospitals for discharged patients who do not have a PCP, a Sound Physicians doctor supports the transition with a virtual telehealth patient visit, places the home health order, and follows that patient for their initial 60-day episode of care. Without this partnership, since patients must be under the care of a physician to quality for home health services, care would be delayed beyond the ideal 24 to 48 hours post-discharge or potentially not ordered at all.
