Texas Home Health and Sound Telemedicine Join Partnership

AUSTIN, TX — Texas Home Health, a leading post-acute care company, and Sound Telemedicine, a division of Sound Physicians, a well-known provider of physician services, have partnered to provide enhanced access to care for patients transitioning from hospital to home health care without a known primary care physician (PCP).

When Texas Home Health receives referrals from hospitals for discharged patients who do not have a PCP, a Sound Physicians doctor supports the transition with a virtual telehealth patient visit, places the home health order, and follows that patient for their initial 60-day episode of care. Without this partnership, since patients must be under the care of a physician to quality for home health services, care would be delayed beyond the ideal 24 to 48 hours post-discharge or potentially not ordered at all.

“This program is a testament to collaboration and commitment of both companies,” said Dr. Anna Loengard, Chief Medical Officer for AccentCare, “and illustrates innovation to address would-be gaps in the care continuum for our frailest patients. This is an ideal use of telehealth in the home and we hope this model will be extended beyond the public health emergency.”

Expediently admitting patients into home health care improves patient outcomes and reduces the risk of emergency room visits, as well as unplanned re-hospitalizations.  Rapid starts of care provide early education, timely medication reconciliation, and assessments of needs or barriers to success in the home environment.

“Because of our foundational belief in integrated approaches to acute and post-acute care,” said Dr. Brian Carpenter, National Medical Director, TeleHospitalist Services for Sound, “expanding our relationship with Texas Home Health gives us a valued opportunity to bridge patient care from hospital to home.”

Texas Home Health and Sound Telemedicine remain focused on quality, satisfaction, and performance. Both companies are very active in telehealth and other innovations to help improve outcomes, increase efficiency, and enhance patient experience.
